How do I have a UK driving licence reissued in my new name?
You need to complete a D1 application form and send it to the DVLA along with a copy of your Deed Poll.
For information and to order a D1 application form, see www.direct.gov.uk or you can obtain this form from a Post Office.
You will need a passport type photo to send with your application form if you are changing from a paper licence to a photocard licence. See form for full details of what you must send.
