What should my notification letter say?
This information is applicable to adults over 18 and people aged 16 or 17 who have applied to change their own name.
If you have changed the name of a child this information is not relevant to you. Instead please see our example Letters of Consent - please select our FAQ heading 'Changing a child's name' for further information.
As an adult, after you have changed your name by Deed Poll you will need to notify all the UK organisations and government departments that hold personal records about you so that they can amend your details and reissue documents where necessary. The booklet we provide with your Deed Poll shows examples of the types of organisation you should notify.
Most will require a letter from you, so we have provided the text below as a guideline, and most will also want to see the Deed Poll. You will need to check with each to see if they require the original Deed Poll, or a certified copy. Occasionally a photocopy is acceptable.
