What is a name change document?

This is a document that proves you have legally changed your name, and shows the reason for the change.

For example if your name has changed due to marriage, a marriage certificate provides evidence of your legal entitlement to your new name. If you have changed your name by Deed Poll, your Deed Poll document provides evidence. Documents that are relevant to other change of name situations include civil partnership certificate, decree absolute, and birth certificate.
