Changing the name of a child aged 15 or younger
If you have Sole Parental Responsibility for your child you do not need anyone else to give permission for you to change your child’s name. When notifying record holders about your child’s new name you will simply show the Deed Poll, plus a letter signed by you confirming that you have sole parental responsibility. An example letter is shown in our FAQ’s under ‘Example letters of consent’.
If another adult shares parental responsibility with you for the child, for example the child’s father, this is called Joint Parental Responsibility and all must consent to the child’s name being changed. Both adults will sign the child’s Deed Poll.
You will be required to state what type of parental responsibility you have on the Deed Poll application that you make for your child, so please make sure that you read about parental responsibility, what it is and who has it, before completing your application.
