What do I do with my Deed Poll document when I receive it?
An explanation of how to execute your Deed Poll correctly is included with your Deed Poll.
First, check that the details shown on the Deed Poll are correct.
Next, sign and date the Deed Poll document in front of a witness, using a blue or black ball point pen or fountain pen. The witness must be an independent individual, not a member of your family or your partner, and they should be able to understand spoken and written English. The witness must be over the age of 18 years and should not live at the same address as you.
Your witness must then sign your Deed Poll document in the space indicated, using a blue or black ball point pen or fountain pen, also stating their home address and occupation.
Your Deed Poll will then be ready for you to use as documentary evidence that you have legally changed your name.

If you ordered certified copies you must send your signed, dated and witnessed Deed Poll back to us in the pre-addressed envelope provided. We will then make your legal copies and forward them to you, with your original Deed Poll.
If you selected archiving, a copy of your original Deed Poll document will be saved in our archives. This means that should you want to order further certified copies at a later date, or if you lose or damage your original document and need to order a replacement, we can easily supply these for you.
